ScripPlus For Wisconsin
Standard Register was selected by the State of Wisconsin Department of Health and Family Services to provide tamper-resistant prescription pads. The agency will provide its pre-approved Medicaid practitioners with the first five ScripPlus® prescription pads free.
Pre-approved Medicaid Practitioners can secure their free, one-ply prescription pads by completing this special order form and faxing it to 1-866-869-3971. Additional prescription pads are available at a discounted price. For questions about ordering, call toll free 1-866-741-8488.
