• "Kentucky Security Prescription" watermarked on backside
• Opaque symbol in upper right corner that disappears if prescription copy is lightened
• Six quantity check off boxes
• Printed across the bottom: "Prescription is void if more than one (1) prescription is written per blank"
• Refill options appear below any logo on left side of blank: "Refill NR 1 2 3 4 5 "
• Preprinted, stamped, typed or manually printed name, address and telephone number of prescriber
• For a controlled substances, space for patient name and address, practitioner' signature and DEA registration number.
