Letter of Authorization
By checking this box, the above referenced company (the "Customer") authorizes Pitney Bowes to provide the Services and agrees to pay Pitney Bowes all amounts owed in connection with such Services. The individual whose name is set forth above represents that he/she is duly authorized on behalf of the Customer to bind the Customer to the terms of this Letter of Authorization of Service.

The current labor rate for non-contract and/or non-warranty Services during normal Pitney Bowes business hours, 8AM-5PM Mo-Fr excluding Pitney Bowes holidays, is $135.00 per hour, one hour on-site minimum, plus travel time, travel expense, parts and $0.36 per mile. On-site time after the minimum, and all travel time is charged in ¼ hour increments. Pitney Bowes warrants Services performed and parts provided will be free from defects in workmanship and materials for thirty (30) days from the date performed ("Service and Parts Warranty"). If the Services or parts do not conform to the Service and Parts Warranty, Pitney Bowes shall re-perform the defective Services or repair or replace the defective part at no charge to Customer, provided Customer has made a claim to Pitney Bowes within the thirty (30) day warranty period.
